*Currently there is inconsistent practice with the data collection log forms.  Sites fax a copy of form to [[Data Processor]] when all patient information is complete on the form. Some staff use various color markers and when forms are faxed, information in row is not visible.  Some staff send the orginal forms in once all discharged for the month are completed and they do not store any copies or original in their office, others staff keep original in office.  If [[Data Processor]] calls with questions related to a variety of errors, collectors can look up patient. The [[Data Processor]] files this information in binders by year, site/unit in main office.
