Anyone do you know? I have used the transfer ready for those that had comfort care and were transferred to the ward but not the ones in the unit that die shortly after they become ACP-C[[User:GHall|GHall]] 16:57, 2015 June 4 (CDT)
**I agree, I haven't been including a transfer ready date for those patients who are changed to ACP-C and end up dying in the ICU. The only time I would add it is if patient lives long enough to be transferred to a medical ward, or if a transfer order is written in the chart.~~
