-This unit is suited to patients who are ready to be discharged but can’t return home to their current living situation this includes patients who are not severely ill or are in recovery phase, exacerbation of a chronic illness, mobility issues including those with non-operative fractures, and end of Life care.   While in transitional care the care team monitors health needs and works with the patient to find a safe, long-term living option in a setting that has the services required to meet their situational needs.
