This article provides information on how to request patient charts from Medical Records at HSC. It is part of the HSC General Collection Guide.

Requesting charts

  • Email Lorna Sasley or take a print out of the medical record request to her inbox at her desk in medical records.
  • Charts have to be assembled before being pulled, and due to the backlog in MR, they will not be available until the following month --Jvelasco 08:56, 2020 February 20 (CST)
  • if charts are from iron mountain, they will take 2 days.
  • For chart review collectors may use the Research and Analysis room (where we normally collect), or if space is too limited for social distancing, there is also another area with separate cubicles that can be utilized. Inquire at the front desk for details.
