The witnessed cardiac arrest on the wiki is speaking only of "in Hospital" arrests when it requires hospital staff to witness.  In the community, I use anyone as a witness.  The only unwitnessed arrests are if no-one is around when the person collapses and no one knows when it occurs and when CPR is delayed because no one could do it.
