*** Just to clarify we are to use the new DATA COLLECTION LOG SHEETS starting the beginning of the month of September/16 however the new sheets do not have the Transfer Ready slot as we had before on the old forms I asked Trish and she said yes we are to include this on the new form So my question is when will the new forms be available to use and who will change it to add the transfer ready DTTM And the new form will need to be included in this section where the old form can be accessed .
