The Data collection log form is a paper form that the data collectors use to write down some info about their patients to keep track of them. Much of the information is identical to what is displayed in CCMDB.mdbs Patient List. These forms "live" for a time being, in a data collector's binder. The Master Log Collection form resides in Main office for print shop.
The sheet is entirely for the convenience of the collector. Please dispose of it in confidential waste when you no longer need it. (The sheet used to be sent to the main office, but this is no longer required (June 8.16))

To see a copy of the Data Collection Log form go to:
Staff are not supposed to use their own version of the Data Collection Logs nor should they be making photocopies of this form. It cost more to photocopy than to get a batch done in the printshop. We currently have a number of versions of the data collection log forms that staff are using. If you are one of those units that are using a version other than one shown in link above, please contact the Data Processor and she will order them from printshop. -Trish Ostryzniuk 14:59, 2012 October 24 (CDT)
Data Collector Use
Keeping track of Serial number / Pat_ID
Shows the last serial used for each ward to show collector which is the next one to use.
- We are discussing if there is a way to do this internal to the laptop. Actually, there is an entry on the wiki for it, must find it. Some of the options discussed today were to keep last 4 weeks of records and summarize to highest and automatically suggest it when starting to enter new patient. Ttenbergen 16:58, 2016 June 8 (CDT)
Previous feedback
coordination between collectors
- comparing admit/discharge times
- sharing serials if that is still done anywhere, I think not... if anyone is still sharing a location and serial pool please post here...
- "One more point, D5 & B3 use consecutive numbers also. These would be hard to keep track of without a paper log for the really quick admissions & discharges that are sometimes seen on B3.--CMarks 13:22, 2012 October 1 (CDT)"
