Background

The APACHE II model to predict death rate for groups of acutely ill patients is composed of the APACHE Score, sets of principal diagnostic categories leading to ICU admission and type of patient as nonoperative or post-operative. Each diagnostic category has corresponding weight in the model.

The list of the diagnostic categories is shown in APACHE Acute Dxs in ICD10 codes and its associated weights are shown in the S ICD10 APACHE Dx patterns table.

There are 30 non-operative diagnosis categories and 24 post-operative diagnosis categories included in the APACHE Death predictive model. These diagnosis categories are further grouped into eight broad categories by Dr. Kendiss Olafson for QI reporting purposes as follows:

  1. Respiratory Disorders
  2. Cardiac Disorders
  3. Post Cardiac Arrest
  4. Neurologic Disorders
  5. Trauma and Poisoning
  6. Post-Operative Care
  7. Metabolic,Renal & GI Disorders, Hypovolemic Shock
  8. Sepsis and Septic Shock

The eight Broader Diagnostic Categories or DxGrp_label are also included in the S ICD10 APACHE Dx patterns table.

Time Reference for month, quarter or year can be based on ICU admission dates or ICU discharge dates. The Critical Report uses the ICU Dispo DtTm

Definition and Derivation

  • Post Operative patient - when the patient's Previous Location prior to ICU is Operating or Recovery Room.
  • The patient based on the Admit Diagnoses and/or admit procedures maybe assigned to one or more diagnostic categories.
    • The diagnostic category with the highest weight is the final diagnostic category assigned to the patient.
