Tiss76 Form Scanning
The TISS Forms are processed by the Data Processor.
The process includes
- mailing to the Main Office
- scanning of the forms
- processing the scan via Teleform
- merging/importing the data into TMSX via <?? is this the downloader app?>
- Tiss forms arrived thru the mail and normally received the next day after they sent there batch.
- Once I received the tiss sorted it by batch the way I appended.
- Then I started writting the study numbers for all the batches appended.While writting I do check the dates and time. Yes they can use pencil to mark the tiss but sometimes the scanners stop me and ask if that is marked or not and also if the pencil is not clear enough scanner wont pick it up.
- Once the study numbers are all written then I started scanning the tiss.
- I normally put 30 or more tiss sheets on the scanner.
- After scanning tiss I verified it right away and appended it on Ed's database.
- After I uploaded it I check quickly one by one the tiss form on the database if there are any extra tiss or missing tiss.
- Then I filed the tiss forms.

Question: is there a way to avoid the need to darken all the bubbles on the tiss form with a marker? University exams have bubble sheets and they just use pencil to complete these and are able to scan them. What is the problem with our scanner? --LKolesar 18:50, 30 July 2008 (CDT) REPLY:
- A pencil can be used as long as the bubble that is being filled out is clearly and visibly marked.
- the "black blobbing" of TISS forms was mandated by the Database Steering Team because the poor quality of the form being submitted by bedside staff.
