"No Transfer Date" in Medicine

  • Started date Feb 29.14 - for all patient still on laptop that REQUIRE a transfer date see Transfer Ready date and time but collector could not guess from chart notes please enter TMP, Project: No Transfer Date.
    • A collector can always give a guess of transfer ready based upon the information in the chart or rounds time, etc. I think we should enter in the tmp if there is no written indication from the doctors when the pt was transfer ready. This way if we want to track this, it could be done. We would then put a transfer ready time,based upon best guess, but a tmp entry would also be made showing that there was no written order or indication from the physician. This way we would know which of these times are just based upon a best guess scenario. --LKolesar 09:09, 2014 March 18 (CDT)

Purpose:

  1. Provide a way for collector to verify that they did not miss entering in this field (left it blank) for those patients that REQUIRE a transfer ready date.
  2. Report back to Director about the number of transfer ready date/times that could not be derived from a patient chart by the data collector. At this time, we are not interested in tallying the number of transfer dates not recorded on chart by the physicians.

  • I just had a look at the validation for transfer date in CCMDB and collectors won't be able to leave this blank is "' if the patient has survived and not left AMA and discharge to is Ward, ICU, home or unknown"; for them to follow this instruction I would need to get rid of this restriction.       • I see that main office is not allowing any ICU profiles without a transfer ready date. Main office follows up with phone calls and emails for a transfer ready date. Tina, ICU collectors should not be allowed to SEND without a transfer ready date. ICU is not using TMP Project No transfer date. Every week Pagasa follows up missing transfer dates for ICU. There are only 3 no transfer date in TMPV2 for ICU to date but transfer date is in registry because main office accepts no blanks. So not sure which are true guesses or just missed. This TMP project will not be applicable to ICU. No one in main office is able to monitor if staff are even remembering to mark it in TMP. TMP Project No transfer date is important for medicine for overstay, because main office if never sure if left blank because missed or could not guess. --Trish Ostryzniuk 18:40, 2014 May 6 (CDT)
    • For ICU, we need transfer dates for this criteria "' if the patient has survived and not left AMA and discharge to is Ward, HSC IICU, home/nursing home/long term care facility or unknown" or the opposite criteria, transfer dates not needed "' if the patient has survived and left AMA or discharge to is another ICU(except IICU) or went to Operating Room ". For ICU, transfer dates for the above scenario (from written order or best guesstimate) are required . JMojica 10:11, 2014 May 7 (CDT)
    • For Medicine, we need transfer dates for this criteria "' if the patient has survived and not left AMA and discharge out of the hospital" or the opposite criteria, transfer dates not needed "'if the patient has survived and left AMA or discharge to is another service such as another Ward (except non-teaching ward), ICU, OR on same or different hospital". For Medicine, transfer dates for the above scenario (from written order or best guesstimate) are needed but if find no clue to make a guesstimate, just leave blanks. JMojica 10:11, 2014 May 7 (CDT)

Need some feedback from data collectors. Are you entering TMP Project: No transfer date when:

  • a. you leave transfer ready blank because could not even guess from chart
  • b. guessing transfer date because nothing written on chart
  • c. both reasons a or b above
    • I think someone should make a decision what is wanted for this tmp entry and then the data collectors will do that. Do you want the tmp entry used even if a guess can be made or in all instances where the doctors do not indicate when the pt is ready for transfer, regardless if a guess can be made or not?? Asking the data collectors what they do does not seem helpful. Tell us what you want and we will do that. (This is my opinion)--LKolesar 06:50, 2014 May 7 (CDT)
    • For Medicine, the project NO_TRANSFER_DATE on the TMP implies having no transfer date to enter because there is no clue found anywhere in the chart to infer the date. This is consistent with item (a) above. If you want to include item (b) which implies having a transfer date but only guesstimate, then the project name should be changed to a different name to avoid confusion. In order to decide which one we need, we have to go back to the basic question 'Why are we doing this and for whom?' Do we need 1) to account how many transfer dates are from written orders or guesstimate or blanks and then report to the management, or 2) to identify the blank transfer dates that were accidentally missed by data collectors (i.e. which will be known if there is no documentation on the TMP). If the goal is (1) then we need both items (a) and (b) but need to have a different project name. If goal is (2), then we need item (a) with project name NO_TRANSFER_DATE.     • I am using TMP for both. The only time I am now using a transfer ready date is if it is clearly indicated when the pt is medically stable, which is very infrequently
