Coding Fistulas in ICD10
A fistula is an abnormal connection between one hollow structure (which may be an organ such as bladder, stomach, or a blood vessel OR a "space" such as the peritoneal space or the thoracic cavity), to another hollow structure OR to the external surface (i.e. skin). We have specific codes for some, but not all fistulas:

Coding instructions
- In general, enter the codes for both structures involved as Combined ICD10 codes -- e.g. for a fistula between the bladder and intestine ("enterovesicular fistula"), combine Intestinal fistula and Bladder, fistula, to anywhere
- When the fistula goes from a hollow structure to the skin (external) then only code the hollow structure itself --- e.g. an anal fistula to the skin just code: Anus or rectum, fistula
- For a fistula involving a hollow structure for which we do not have a specific fistula code, then use the appropriate NOS code -- e.g. for a fistula involving the gallbladder, use Gallbladder or bile duct disorder, NOS
