There was talk about comparing Postal Codes to known PCH Postal Codes. Since these might Include other buildings at the same site that are not PCHs, this check can at best be a soft check. Please add the list of these postal codes here.

Background

Julie found cases where the pre acute living situation for direct admits are not consistent with Previous Location and/or Dispo. We want the DC to be aware that the pre acute living situation has a relationship with the Previous Location or Dispospecifically for PCH cases.

Examples:

I rely on the combination of pre acute living situation; Previous Location; Postal Code and Dispo fields to identified new or already PCH residents prior to admission. Data collectors must understand that Assisted Living and Supportive Housing are not PCH and must be entered as simply HOME as confirmed by Dr Roberts.

As per email from Julie:

I have asked Pagasa to audit some records where the postal code matches with the PCH Facility.  In most of these cases, Pagasa found that EPR addresses (no and street) match with the PCH facility’s addresses but the Pre-acute living situation =’Apartment’  instead of ‘Personal Care Home’ .  this is possible because the address has the same format   ‘Suite# - No and Street Name’ .  

PCH resident data prior to admission is not only being used in Medicine but also needed  in Critical Care. 

Tina, is it possible to ask at entry level,  say, – the postal code matches with a PCH facility (show the PCH address) but the pre-living situation entered is not ‘Personal Care Home’ , is this correct?    I suppose DCs are getting the postal code from EPR where the exact address is also visible.

  • from a data perspective, what do you mean by "admitted directly"? If I were to build a check, where would I find that? OR maybe I don't need to know, but then I need to have a definition of what combination of data would be an error.
