This page is about the Cognos Admitter form which lists Cognos EPR Report data for patients who have been admitted to the laptop's hospital and program within the last two weeks. It can be filtered to only list a specific service. It has a button to open create, copy or open those records on the Patient viewer tab Cognos ADT to facilitate entering their admission info. The form can be opened from the Main Form (CCMDB.accdb).

Instructions

Use the Binoc button.png pick the Cognos line you want to work on;

If the record is on the list but doesn't require an entry either in L_Log or in Boarding Loc, you can use the "exclude" button to make it disappear from this list and future versions. You can use the "View exclusions" button to see a list of what you previously excluded.

Use the service dropdown in the top row to only list patients for a specific service. Click the ""Filtered" button at the bottom of the screen to list all services again.

What data is included on this list

See Cognos_EPR_Report#What_data_is_included for info on what data is included in the report we receive. The Cognos Admitter further filters this data to not show lines that are clearly related to a record that has already been deleted from the laptop, and it only shows lines for the location of the laptop. The form has further filters to show even less data as convenient.

Filter by patient

If you click the name of a patient, the list will be filtered to only records for that patient at your site and service, sorted by Cognos Unit Arrive Dtm. This should make it easier to figure out which records are Boarding Loc moves (in which case, use the binoc button of the line that is their actual admission to open the record and add the boarding loc entry from there).

Click the name again to see the full list again.

  • I realize now that this will be a problem when the initial admission is not within the last two weeks. I will need to figure out how to deal with that for those records. Please work around that for now, e.g. by opening the pt in Patient List and adding the tmp entry from there. Once done the line will show up as a boarding loc line in Cognos Admitter.

Improvements needed?

Records on list repeatedly

  • The patients that have already been entered on our laptop, but are being transferred back to our units, requiring a second or third admission are hard to pick out/decipher. It would be helpful to have a clear "previous location", next to an "arrive time" field on the Cognos Admitter screen, so that we could easily figure out what we are supposed to be doing with that patient. These patients are also showing up multiple times in the Admitter, adding to the confusion. I'm not sure if there is anyway to get rid of the multiple lines showing the same information?
    • Agreed, this is something that needs to be improved still. I can show previous unit or service from the Cognos data for anyone, and previous location data for patients already entered. I think the real confusion here is that we still see too many lines right now: if there are two records in cognos and two in L_Log, right now you get 4 lines as it compares (a-A), (a-B), (b-A) and (b-B). I am working on a way to not show (a-B) and (b-A), which will improve this. One of my problems is that some of the records will match on a Boarding Loc entry, not an Arrive DtTm.         • There are less duplicates, but on a couple of patients that moved from two different units that I collect on, there was no peach color allowing me to enter a new profile on the patient. I don't know if this is because I entered the dispo time on profile #1 prior to creating profile #2, but even when I went in and deleted the dispo information, the two lines still showed up as light green. I then went into my CCMB and entered a new profile for that patient using the patient copier button, at which time I had four lines for that patient in my admitter, one of them is now peach. Not sure how to better explain this unless I am showing you as it is happening...
          • I changed the colours and how they are generated pretty much completely (two buttons now); please give it a try now and let me know if it still misbehaves. Ttenbergen 23:01, 2020 June 29 (CDT)

Picking out transfers

  • The transfers out of the unit are also difficult to appreciate/easily pick up with the current layout.
    • I would expect you to find the transfers out in the Cognos Discharger instead of the admitter. And if you deal with them in there, you should not have to deal with them in here. Caveat: I am working on a way to deal better with the Boarding Loc entries.
      • Cognos Discharger should be an easier place to deal with transfers; I would always deal with discharger first. And Patient viewer tab Cognos ADT is an easier place to deal with tmp/boarding transfers, so if a record looks like it should have boarding records, open it from Admitter and then do the actual additions in there. The color indicator where a tmp already exists should be fixed now. Ttenbergen 23:01, 2020 June 29 (CDT)

  • We have had patients transferred from MICU to a boarding location that are not showing up in the admitter, because on transfer they are still under critical care medicine, they are changed to medicine service at a later date. Will they show up on the admitter when their service is changed from critical care to medicine service? if not these admissions will be missed. Lisa Kaita 13:50, 2020 July 13 (CDT)
    • That is an interesting scenario. To start with, are we talking about the admitter or the discharger where these are not showing up? Then: I think right now the discharger only shows lines that have a unit end date; should it also show lines that have a service end date, then? It would be an easy change, it would just mean more lines you don't need to deal with showing up in discharger. Is there anything else that we would be able to filter on to only show service changes when they make a record relevant to our discharges? For example, would this be only if someone is on a unit where we don't collect? And, have you had a look at the raw data, are the lines actually in there? This might be a niche case where they are not, so good place to start. Ttenbergen 16:51, 2020 July 15 (CDT)

Automatic serial numbers not working as expected

The New Pt Serial helper form was updated to always list the hightest serial on the laptop + 1. See Facilitated Management of Serial numbers for more info and discuss any concerns there since this also applies to how pt is entered from Patient List, not just from Cognos.
