Data collection log form

The Data collection log form is a paper form that the data collectors use to write down some info about their patients to keep track of them. Much of the information is identical to what is displayed in CCMDB.accdbs Patient List and Previous entry form.

These forms "live" for a time being, in a data collector's binder. The Master Log Collection form resides in Main office for print shop.

The sheet is entirely for the convenience of the collector. Please dispose of it in confidential waste when you no longer need it. (The sheet used to be sent to the main office, but this is no longer required (June 8.16))

Staff are not supposed to use their own version of the Data Collection Logs nor should they be making photocopies of this form. It cost more to photocopy than to get a batch done in the printshop. We currently have a number of versions of the data collection log forms that staff are using. Data Collector Use

Keeping track of Serial number / Pat_ID

Shows the last serial used for each ward to show collector which is the next one to use.

  • We now have Facilitated Management of Serial numbers. This should eliminate the function of the paper forms to keep track of serials. If it doesn't, could someone please explain how the paper log is still helping with this? Ttenbergen 09:11, 2020 April 15 (CDT)
