The data in the CCMDB is frequently used for Quality Improvement purposes and to facilitate other research.
All access to the data our database needs to be vetted. To apply, a Database Request Form needs to be filled in. Trish and Julie can provide this form. If you have access to the regional server the form can also be found at:

- We need to generate a PDF of these and upload of the data request form
- bitmap copy of database request form?? Suggestions??
- I think this section should refer to . That section still would need access to the form, though. Do we have a PDF version of the form? If so, we should be able to link that in there. For the time being, that page refers to the regional server for those who have access to it.
