Database Request Process

The data in the CCMDB is frequently used for Quality Improvement purposes and to facilitate other research.

All access to the data our database needs to be vetted. To apply, a Database Request Form needs to be filled in. Trish and Julie can provide this form.

The master copy of the data request form is stored at

• Regional Server\Admin\Data_Request_Forms
