Additional Info
- This code only applies to people with diabetes mellitus
- It is a generic code for the acute complication of diabetic coma, which can be due to hypoglycemic coma, hyperglycemic coma, or Diabetes mellitus acute complication: Non-ketotic hyperosmolar state
Diabetic hyperosmolar conditions
- We have the following codes that can be used to code various diabetic hyperosmolar conditions:
- Diabetes mellitus acute complication: Non-ketotic hyperosmolar state
- Diabetes mellitus, in poor control
- Hyperglycemia -- this is an abnormal test result code, not a disorder. Typically for persistent hyperglycemia in a diabetic you'd use Diabetes mellitus, in poor control instead
- Diabetic coma -- this code covers any type of coma due to diabetes: hypoglycemic coma, hyperglycemic coma, or coma in Diabetes mellitus acute complication: Non-ketotic hyperosmolar state
- They can be used in combination as appropriate -- e.g. to distinguish the cause of hyperosmotic diabetic coma, you can combine Diabetic coma with:
- Diabetes mellitus acute complication: Non-ketotic hyperosmolar state -- for non-ketotic hyperosmolar coma
- Diabetes mellitus, in poor control -- for hyperglycemic/hyperosmotic coma NOT due to non-ketotic hyperosmolar state
Altered mental status coding guideline
Coding altered mental status in ICD10 can be complex, see ICD10 Guideline for coding altered mental status for more info.
Diabetic Hypoglycemic Causes
- For diabetic coma of the hypoglycemic variety where the hypoglycemia is due to an acutal overdose of a diabetes drug, combine this code with Insulin or other antidiabetes drug, overdose/toxicity
- For diabetic coma of the hypoglycemic variety where the hypoglycemia is due to another cause (e.g. not eating while continuing to take the usual dose of insulin), then combine this code with Hypoglycemia, in diabetes
Alternate ICD10s to consider coding instead or in addition
- Somnolence, stupor or obtundation
- Coma NOS
- Insulin or other antidiabetes drug, overdose/toxicity -- this code includes hypoglycemic coma in a non-diabetic
- Myxedema with coma
- Hyperglycemia
- Hypoglycemia
- Hypoglycemia, in diabetes
- Cerebral edema
- Myoglobinuria
- Dehydration (volume depletion, hypovolemia)
- Hypokalemia, severe or symptomatic
- Acidosis, NOS
