I did not do the analysis of this, only Basil so I do not know the time period.  I just saw in the shared folder the 2018.  and you said you did the lime survey in 2020 - no 2020 data in the shared folder too. The Survey - Family Satisfaction in the Intensive Care unit (FS-ICU) is a survey that the ICU Quality Improvement Team (QIT) would like to run periodically for three months.

Survey Dates

First Wave 2011

  • Date: May to Nov 2011
  • Quality Officer : Brenda Kline

Second Wave 2015

  • Date: April 1 to June 30, 2015
  • Quality Officer : p:Basil Evan

Third Wave 2017

  • Date: Jan 1 to March 31, 2017
  • Quality Officer : p:Basil Evan

Fourth Wave 2018

Sampling Plan / Procedure

Inclusion

  • FS-­‐24 Surveys are distributed only to families of patients with ICU length of stay > 48 hours

How?

  • Surveys are given to families of patient around 48 hours from ICU admission with follow-up reminder at the time the patient is transfer ready, prior to ICU discharge.
  • Letter of condolences and survey forms are also mailed after 4 to 6 weeks to families of patients who died in ICU.

Sample size and Response Rates

  • 2011: Distributed =618 Received back =191
  • 2015: Distributed =346 Received back =139
  • 2017: Distributed =414 Received back =191
  • 2018: Distributed = ?? Received back =151

Data located

  • All data are saved separately by year in ACCESS databases on S:\Critical_Care\Patient_DB_Reports\FamilySatisfactionSurvey\Database.
  • Each database name is Survey_Database_yyyy.mdb where yyyy represents year (i.e. 2011, 2015, 2017)

Survey Forms

  • Completed Survey forms - are kept by the Quality Officer.
  • Blank survey form is located in S:\Critical_Care\Patient_DB_Reports\FamilySatisfactionSurvey\Forms\Family Satisfaction Survey.4 pages.pdf.
  • The survey form with entry codes is located in S:\Critical_Care\Patient_DB_Reports\FamilySatisfactionSurvey\Forms\FamilySatisfactionSurvey_CodingForm_2017_18May2017.

Data entry

  • 2011 - Karen, the admin person for Linda Hathout's Quality and Process Team, did the manual entry in an Access.mdb.
  • 2015 - The Quality Officer collected the forms and did the data entry in access.mdb
  • 2017 - The Quality Officer collected the forms and did the data entry in access.mdb

Data Analysis and Reports

  • p:Julie Mojica made frequencies and bar charts for each of the items of the survey form
    • The SAS program for the frequencies is in S:\Critical_Care\Patient_DB_Reports\FamilySatisfactionSurvey\SAS_Julie\PFSurvey94_prep.sas
    • The SAS program for the bar charts is in S:\Critical_Care\Patient_DB_Reports\FamilySatisfactionSurvey\SAS_Julie\PFSurvey94_chart.sas
  • The tabular report is on S:\Critical_Care\Patient_DB_Reports\FamilySatisfactionSurvey\Report

  • The tabular results and charts are also provided to p:Dr. Kendiss Olafson for the Quality Improvement Team (QIT) at shared folder O:\FSS.

History

The designed Teleform survey form was lost for some unexplained reason in 2011 when moving Teleform program to newer computer. For some reason it could not be recovered from old computer either. That computer is RIP as of 2011.

2011 - done in collaboration wit Critical Care Vital Signs Monitoring & p:Dr. Kendiss Olafson-Quality and Process Improvement Team & previously Brenda Kline- ICU Quality Officer.
