The designed Teleform survey form was lost for some unexplained reason in 2011 when moving Teleform program to newer computer.  For some reason it could not be recovered from old computer either.  That computer is RIP as of 2011.

The Survey - Family Satisfaction in the Intensive Care unit (FS-ICU) is a survey that the ICU Quality Improvement Team (QIT) would like to run periodically for three months.

History
