L Log not updated for location, sex and type
Sending runs two queries, first an update for any existing one and then an append for new ones. From the start of sending until 11:28, 2014 September 3 (CDT), the new/append one included the following, but the update-forexisting one wrongly did not:
This would have resulted in patients for whom data in these fields was changed after the first send to have faulty data in centralized_data.mdb because it would not have been updated. The bug was found when Pagasa flagged blank R_Sex fields in complete data, which caused a review of the update query, which brought up the other two fields.

Query send_Centralized_1L_Log_update fixed in CCMDB.mdb_Change_Log_2014#2014-Aug-25. Future sends will update.

All options to find and fix would have been done and picked up except for type field. I remember fixing type field when Julie provided a list. Example from OR but Medical etc. This was awhile ago and I believe you fix the issues after your reported and we check. Just never go cleaned out of here.
