Legacy Process, stopped all forms late 2020 and early 2021
Data Collector instructions
Form that must be ordered from printshop through the Data Processor in the Main Office. Email p:Pagasa Torres directly with your request.
Do not photocopy forms at your site. It is more cost effective to order forms from printshop here.
