Old Code Pneumothorax-iatrogenic

This code took over from Pneumothorax-iatrogenic at some point, but the old code was not disabled. Both likely exist in the data. The old code was replaced with this one because this one includes specific reasons for iatrogenic complications. ICU had wanted to track these.
