ACTM to disable accounts and removed from global address book. Termination

As per email from Sandi Kroeger to Tina 2015-07-16: 
Use the usual Account and Access Request form. For type of request, 
select 'modify existing account', provide full name and userid.

In the Additional Information box, type in that we are to remove 
only her Data Collector Role access, and make sure to check the 
applicable application boxes if you know which ones they have access to in other jobs. (I.E. PACS).
If they retire, then remove all access to all applications.
