TISS Form (TISS28)

The TISS Form for TISS28 is used as part of TISS28 Data Collection. See TISS General Info for general information about our TISS data, and TISS28 Collection Guide for details about how our Data Collectors collect this.

The Form

The current TISS form can be found at Media:TISS28 Form.pdf.

Ordering copies from the HSC print shop

Master forms are reproduced at the HSC print shop. To order copies:

  • Complete a print shop requisition. The following should be specified:
    • Cost centre (Critical Care)
    • Quality of forms required
    • Print on pale pink paper
    • Print double-sided (duplex)
    • Hole punch (3-hole)
  • since we no longer keep these, do we need them hole punched? Ttenbergen 11:37, 2017 December 12 (CST)
    • It may well be cheaper to not have these punched, can we stop the holes?

  • Forward the completed requisition to the print shop (keep the pink copy for office records).
  • Email the master PDFs (in X:\TISS28\Forms\For Printshop) to the print shop, referencing the requisition number.

QA form check by data processor

When the printed forms are returned from the print shop, perform a quick quality assurance check before distribution to the sites:

  • Is the print quality acceptable?
  • Are the holes punched on the correct side?
  • Is the form image aligned correctly on the page?

Data processor Admin task

  • Mark the date received in main office on the original requisition.
  • Also indicate the date the copies were distributed to the various sites.
  • File the completed requisition.

Form Design and Implementation

We use Teleform for TISS28 Form Scanning so the form has to be exactly the one that was generated by Teleform Designer. Any modifications of the form could break our scanning process.

The master copy of the form resides in Teleform Designer on the Data Processor PC. Production Version 2012-Dec-11 is the "master" copy used to evaluate completed and returned forms when scanned.

Printshop version

  • The Printshop paper Version dated 2013-Mar-5 is using Teleform production Ver 2012-12-11. Wording changes have been made on form for printshop for items #2,8,11,31, 39. Top of form instruction made larger also. Revision information at bottom of form updated.
  • The electronic copies of TISS28 form for each site for printshop in located on X:\TISS28\Form\PDFs_for_Printshop\2013_Mar_05_for Printshop. Refer to this copy of TISS28 of TISS28 printshop form for reference only.

Preparing site-specific masters

There are site-specific PDF templates for each ICU site in X:\TISS28\Forms\For Printshop

As an aid to data collection, each site is provided with blank forms that are prefilled with their respective site designator, e.g., HSC, GRA, etc. To prefill the site designator:

  • Open Teleform Designer.
  • Open the TISS form template (TISS28 PROD 2012-12-11).
  • Double-click the SITE field. Teleform will display the field properties window.
  • Click the Appearance tab.
  • Click the Prefill tab.
  • Enter the three-character abbreviation for the site to be prefilled (one of HSC, GRA, VIC, STB, CON, or OAK).
  • Click OK
    • IMPORTANT: DO NOT press save when you have finished the above task. This will affect the PRODUCTION that is already in production.

The site designator will now be pre-filled on the form.

Creating the PDF

There are site-specific PDF templates for each ICU site in X:\TISS28\Forms\For Printshop

To create a PDF of the prefilled form:

  • Select File | Print from the main menu.
  • Select Adobe PDF from the list of available printers.
  • Open the printer properties window.
  • Select High Quality from the list of available settings.
  • Click OK.
  • Click Print.
  • Save the PDF to X:\TISS28\Forms\For Printshop.

Repeat the above steps (prefill and print) for each site. Close Teleform Designer after a master PDF has been created for each site. If Designer prompt to save changes, click No.
