This is not going to start any time soon, I just need a place to park the notes I took in a task meeting a while ago.

Collection process

We would document problems in local nurses filling TISS forms so we have quantitative data on error rates.

Data Collection Instructions

For some sampling of critical care patients, enter the following.

Sampling rate, start date and other things to be determined.

Enter the following in tmp:

  • Project: TISS form audit
  • Item: as appropriate, a line for each
    • not done for day
    • more than one item done for day
  • Date: first day of each instance of a problem (ie if they lost two forms during stay, enter two lines)
  • Notes: if you learn why the problem happened, note here
