Patient Profile

This unit is suited to patients who are ready to be discharged but can’t return home to their current living situation. This includes patients with an established diagnosis that does not require further investigations, low risk for rapid decline, behavioral and neurological difficulties that can be managed with an established care plan, reconditioning, and discharge planning.

While in transitional care the care team monitors health needs and works with the patient to find a safe, long-term living option in a setting that has the services required to meet their situational needs.

    • patients admitted to this unit do not have a Family Physician or whose Primary Care Provider does not admit to a Sub-Acute Unit. Admitted under an Attending Family Physician/GAP - Family Medicine.

important note: -May to July 2018 renovations, pt physical location and EPR location label

  • On occasion N4C extension beds are opened: 418-3 to 419-4 = 6 beds
  • MAY 1/2018: N4C patients/staff will be temporarily relocated to the S4 Unit during N4C renovations.
    • Estimated timeline end of July 2018.
  • IMPORTANT: These beds appear on EPR as NU4S however these rooms are part of N4C and are under VGH Medicine-Trnsnl/General which = Family Medicine Service.
  • S4 is closed for renovations with plans to reopen as a Geri Rehab Unit.(presently located on S2 ward)and will open as Geri rehab once N4C renovations are completed
  • code location as N4C do not check OFF WARD Wards will appear as S4 417-1 to 436-2 code all S4 wards as N4C til renovations are complete .
    • July 12, 2018-Unit renovations complete and patients relocated back to N4C (Rooms 401-418-2)
