These are patient Admission/Transfer/Discharge (ADT) paper log forms that "live" on individual units & wards and can be found in ward ADT log books. These forms are never collected by the collectors or sent to main office.
Wherever multiple collectors share collection duties for a ward, they will write on these ward logs with various tags to help keep track of who collected on which patient in that ward using what serial number was used.
The ward admission logs forms are totally different from Data collection log forms, (aka PDA Collection Log forms). These forms "live" for a time being, in a data collector's "collection binders. -Trish Ostryzniuk 17:48, 2012 October 18 (CDT)
