2017-Oct-10: VIC_N4 became [[VIC N4C]]-transitional care.

*Some patients were still on the unit when N4 converted to N4C on this date. Medicine directors wanted to account for these, even though we did not use to collect on this ward therefore, patients who were already there will be collected as VIC_N4 until they are discharged.
