*please note after entering patients in the transition coordinator book, there is an ~~orange sheet taped to the wall by the door, please write the room number of the new "red" patient with the date and allied health service responsible for following the patient~~

*the colored dots are kept in ~~the front~~ of ~~the TC binder, when running low, please let Leah know and she will reorder~~

*Charts are located in the physicians room, right next to the nurses conference room, on rolling carts

*thinned/old charts are located next to the active charts on a ~~rolling cart~~
