Osteomyelitis must meet at least 1 of the following 3 criteria:
 
1. Patient has one or more [[pathogen]] identified from bone
2. Patient has evidence of osteomyelitis on gross anatomic or histopathologic exam
 
3. Patient has 2 or more of the following 5 signs or symptoms [fever>38.0, swelling, pain/tenderness, heat, drainage]

:has positive blood culture OR imaging test evidence suggestive of infection (for example, x-ray, CT scan, MRI, gallium scan, technetium scan)
