Historically, the data collectors have been provided with a '''collection worksheet''' to document procedures and treatment information. This information was scanned in for future reference and for simple clarification of diagnostic coding.

Some locations are still using these sheets, others are using a modified version, ~~and some~~ enter data straight into the ~~PDA~~.
