ACP Status Collection: Difference between revisions

Discussed this with DR today; sure enough I asked about only the example of a previous service, but didn't explicitly consider a patient returning to our service after. So the "definition" part above is a bit of an extension I just decided on the spot when I realized his answer would create more questions.
