Non-standard CCI Procedure

This is a list of the non-standard CCI codes that were added to our list, and the reasons.

CPR, cardiac resuscitation	Until 2019-07-24, the code for this was "1.HZ.30", which is the same as the combo code for Heart, and ultrasound. The data can be clearly identified by checking the tables directly, but the query Julie uses to combine the contents of the combo and picklist CCIs would list them with the same code. So, we added a "r" (for resusc) to this code to make it distinct. All old entries that were initially entered without the "r" have been changed.
Debridement	Added an "d" for debridement to distinguish from the other "87" code Excision, Resection; debridement would have usually been part of that code, but to allow it to be collected in CCI Collection Mode "CCI collect first" it needed to be a separate code.
ECMO, VA	The CCI ECMO code 1.LZ.37.GP-QM doesn't distinguish between VV and VA, and we need to know.
