Collection staff port around with their laptops, a data collection binder which contain the follow items:

  • Data collection log forms - ICU and medicine collectors
  • own history note worksheets per patient - ICU and medicine collectors
  • completed TISS28 forms - ICU collector
  • Green sheets - ICU use only, but should be disposed of after collecting and reviewing.
  • Medical records lists
  • other work related items
  • some people keep a BMI chart in their binder for the determination of obesity or morbid obesity. We could just google this instead or put it on the wiki.
