The Data collection log form is a paper form that the data collectors use to write down some info about their patients to keep track of them. Much of the information is identical to what is displayed in CCMDB.accdbs Patient List and Previous entry form.
These forms "live" for a time being, in a data collector's binder. The Master Log Collection form resides in Main office for print shop.
The sheet is entirely for the convenience of the collector. Please dispose of it in confidential waste when you no longer need it. (The sheet used to be sent to the main office, but this is no longer required (June 8.16))

Staff are not supposed to use their own version of the Data Collection Logs nor should they be making photocopies of this form. It cost more to photocopy than to get a batch done in the printshop. We currently have a number of versions of the data collection log forms that staff are using. The Previous entry form makes available some information about previously entered and deleted patients - no names, but chart numbers are in there. Would that be good enough to replace the paper form?
