This patient was admitted to 3 South from ER and when they transferred the patient to 5 North they had to temporarily (in EPR) add the patient to the SWAP location before finally completing the transfer (in EPR) to the new boarding location on 5 North.
 
This can also happen when a patient needs to move rooms on the same unit (for example from a regular room into an isolation room). You'll see the patient entries as listed above, only the new location after the SWAP entry will be the same as the location they were coming from. In this case, nothing new to enter because their physical location has not changed.

The EPR/Cognos unit entry GH-SWAP Location is used when a pt is swapped form one bed to another.

Collection Instruction

  • There is no physical SWAP location, and is used in EPR when they are changing patient bed/unit assignments. We don't enter Swap locations, but will enter the move in the Boarding Loc if the unit has changed. If someone remains in the same unit but moved rooms, nothing is entered.
  • In COGNOS, it will show up under the Unit as GH-SWAP

  • In the 40 days preceding 2020-12-15 there appear only 2 of these entries and they are about 1 minute each.
