*Yes,there is no reason to have 3 views for the MR list. Please just put one list and just label it "MR list" (the oldest first list should be used). Also, for the format of the form, please put the MR # first, then the pt's last name, then the first name, then the dates, etc. The serial # is OK to be on the form but if you can put it later it would be better.
