Patients who are declared medically incompetent can have much delayed discharges so we want to track when this happens.

            Certificate of Mental Incapacity

Who Fills: Must be Attending MD Is Psych involved: Not an absolute requirement-some Physicians prefer not to consult psych but when/if they are-it is a written assessment in chart-not on the form. If patient/client already has a POA [Power of Attorney] for any reason and becomes incompetent then the form is not required as they can initiate any/all decisions arising from this determination.-Louise
