** What we are hoping for from our end is to understand when you guys last reviewed a file. ***I think that it should be something that the data collector initiates when she is physically updating a chart...such as clicking on a button. We often open up our patient files to enter a lab value, clarify a diagnosis, add something to the note to remind us to check on something in particular, etc. Otherwise we will think that a chart has been updated when in actuality it has not been touched for quite some time.
