**HSC's pink sheet is only seen rarely. Usually I've only seen it for patients who came in for a same day procedure and then ended up needing to be admitted to medicine. This is a temporary project to track source collectors use for Accept DtTm.

Operation Planning - Patient Flow. Region Managers and Director are monitoring and tracking patient flow from the emergency department to their unit. The data helps support any changes needed to improve patient flow from ER to unit.

We currently track Accept DtTm however, we need to identify the "source" that this information is being abstracted from by collectors in Region. We are starting a short term project in order to do this.

Collection instructions

Applies to all patients admitted from your local emergency department only. When you enter an [[Accept DtTm field | Accept Date/Time]}:

  • TMP tab on laptop in CCMDB.mdb
  • Project: AcceptDateSource
  • Item: select all sources this info was documented at your site. Could be one or more with different or same DateTm

Template:Discussion before we start, I need help for the drop down list for items in regards what the sources might be. This is tentative list subject to staff input. I have started the list below:

  1. ER form-Request for Admit to inpatient unit

    • column E - date
    • column M - time
    • column B - check box - mark which source you used in Dispo tab for AcceptDtTm.
    • Column Q - COMMENT - if guess or other

"ER Department - Request for Admission to Inpatient Unit" form

At HSC, Director of Medical Records states that there should be a white form on chart - ER Department - Request for Admission to Inpatient Unit. (Trish asked them to fax her copy). Hospitals using this:

  • HSC
  • Vic
  • Conc has this form but is not a permanent part of the chart,it is tossed in confidential waste.

VIC consult sheet

  • the consult sheet in ER/viewed in EPR filled out by the Bed Doc (VIC) will indicate whether he will admit to Medicine(teaching vs Nonteaching) however we dont use that time only the Pink Inpt summary Sheet time.
