** Would it make sense to just notify the coordinator with this info ASAP when the VAP/CLI happens? That's what we do at STB, we use a button right in the program for it so there is very little overhead for collectors. Collection staff port around with their laptops, a data collection binder which contain the follow items:
- Data collection log forms - ICU and medicine collectors
- own history note worksheets per patient - ICU and medicine collectors
- completed Greensheets - ICU collector
- completed TISS28 forms - ICU collector
- Medical records lists
- colored stickies for medicine OverstayProject - Medicine collector
- other work related items
