Sometimes patients are admitted under a stolen identity, and occasionally this is noticed and affects the patient's chart.
Collection Instructions
- Put the real name if available with the real DOB, real Chart number and PHIN in the Database entries and put the stolen ID in the Alias ID collection.
- If other admissions were recent and part of our database, contact the collector or main office to sort this out as an Alias ID collection for previous admissions. Also let Pagasa know if the wrong name was initially used in the profile.
- The hard part is confirming the stolen identity and being able to determine the real identity.
