This page contains instructions that had been duplicated on HSC Medicine Collection Guide and HSC Critical Care Collection Guide. They were moved her to make sure both sets of collectors use the same instructions.

Dialysis Form
Patients admitted at HSC and receiving hemodialysis treatments will have a copy of their treatment record placed in the flow sheet section of the in-patient health care record. As of 2016-03-22 they no longer have a HD note written in the IPN section of the chart following their treatment.
