This entry is about the time of an intent, nothing to do with what actually happened to the patient after.

*If the patient is "medically stable" (written anywhere in the chart) and/or care is stepped down ie. VS decreased, off monitor, IV meds changed to PO meds, etc. then the patient is "transfer ready". For instance a patient may be very deconditioned but still be medically stable. This can be decided by our nursing judgement as we follow the patient through their hospital stay on our wards. This was discussed at a previous task meeting and Dr Garland agreed that it can be decided by data collectors. --CMarks 14:39, 2017 January 13 (CST)

**If a patient is made ACP-C in ICU they are considered "transfer ready" regardless of whether they or not they are still intubated and ventilated according to Dr Garland at Task meeting on Jan 12, 2017.--CMarks 12:12, 2017 January 23 (CST)
what is transfer ready?
*When there is a  plan to send a patient to IICU at HSC : for transfer ready date and time, use the date and time that the IICU consult is written and the patient is ready for transfer to that unit. Often there is a long wait for a bed to become available before actual transfer can occur. The transfer ready date and time helps to reflect the long wait times for beds in IICU. --LKolesar 12:27, 2017 January 20 (CST)
Any of:
* "medically stable" written anywhere in the chart
* care is stepped down e.g. vital signs decreased, off monitor, IV meds changed to PO meds
* patient is made ACP-C in ICU, regardless of whether they or not they are still intubated and ventilated
* date and time that the IICU consult is written for patient going to HSC_IICU

* A deconditioned patient may still be medically stable if they can re-condition elsewhere; use your judgement.
