River Ridge Transition Care Environment (TCE)

Clients transferred to the TCE are medically stable and ready for discharge from hospital, but still need time to make long-term arrangements for care. In the TCE, the care team assesses and determines the level of care and support necessary for each client.

Following a TCE stay, clients may be able to return home with home care supports, be eligible for supportive housing, or PCH.

River Ridge can be collected as Previous Location or Dispo.

Medicine program wants to track dispo to or from TCE. Template:Discussion for the purpose of what? Will this be in reports? Which ones?
