River Ridge Transition Care Environment (TCE) River Ridge Transition Care Environment (TCE)

  • Patients transferred to the TCE are medically stable and ready for discharge from hospital, but still need time to make long-term arrangements for care. In the TCE, the care team assesses and determines the level of care and support necessary for each patient.
  • Following a TCE stay, patients may be able to return home with home care supports, be eligible for supportive housing, or PCH.

River Ridge can be collected as Previous Location or Dispo.
