Local swipe card for facility access

To get the form:
- in your outlook go to public folders
- select all public folders
- go to Security Services folder
- See Facility Access form
- must be signed by: Manager
- Room MS245
- M-T 9-3, Fri 9-12

This is info we might best keep in collaboration with the Internal Medicine folks, as it should be the same and we could learn from each other / prevent duplication. would it be OK to move this to with a link from here?
