The Lennox Bell Project is for discharged patients from HSC med wards to Lennox Bell Lodge. These are not patients on hospital hold but are individuals that need interim housing for a short period of time.
Any HSC medicine ward patient that is DISCHARGE TO Lennox Bell is coded as: HB
Data Collectors will not be expected to follow these patients during their stay in Lennox Bell.
