Started: September 16.13: The Lennox Bell Project is for discharged patients from HSC med wards to Lennox Bell Lodge. These are not patients on hospital hold but are individuals that need interim housing for a short period of time. Any HSC medicine ward patient there will coded as DISCHARGE TO: HB Data Collectors will not be expected to follow these patients while in Lennox Bell.
