Definition: Palliative Care Service for Medicine and ICU database program: means the patient is either under the care of Palliative Care Service team or the Palliative Care Service Team has been consulted. It will no longer mean comfort care, ACP C, palliation. Go to: ACP C, or Comfort Care for collection instructions on that care goal.
Coding Instruction for Palliative Care Service
Palliative Care Service code should only be used as follows:
- ADMIT slots:
- only if the patient has been previously enrolled in palliative care service and they are involved in their care.
- COMPLICATION slots:
- only if there is an actual "consultation" for Palliative Care Services AFTER ward admission.
NOTE: change in collection process
- If patient care goal at the start of a Med or ICU unit admission as per physicians admitting note is palliation, comfort care, ACP-C and the patient is not under Palliative Care Service, then go to: ACP C, or Comfort Care for further collection instructions.
- If patient becomes ACP C, or Comfort Care AFTER ward admission by the attending physician or resident, you cannot code Palliative Care in complications anymore, unless the Palliative Care Service is consulted.
